Position Overview:
United Woundcare Institute is seeking a dedicated Patient Advocate to join our team. In this role, you will work closely with patients to guide them through the complexities of healthcare programs such as Medicare, Medicaid, and Social Security. You will also assist national patients in transitioning in and out of wound care plans, ensuring they receive the comprehensive support they need.

Key Responsibilities:

  • Assist patients with the enrollment process for Medicare, Medicaid, and Social Security.
  • Provide one-on-one support and education to patients about their healthcare options.
  • Coordinate transitions for patients entering and exiting wound care plans, facilitating communication with healthcare providers.
  • Develop and maintain strong relationships with patients to ensure their needs are met effectively.
  • Advocate for patients' rights and assist them in understanding their benefits and entitlements.
  • Collaborate with interdisciplinary teams to ensure holistic care for patients.
  • Stay updated on changes in healthcare policies and programs to provide accurate information to patients.

Qualifications:

  • Bachelor’s degree in a related field (e.g., healthcare, human services). Social work background is preferred
  • Experience working in a healthcare setting or in a patient support role is preferred.
  • Strong understanding of Medicare, Medicaid, and Social Security programs.
  • Excellent communication and interpersonal skills.
  • Compassionate and patient-centered approach to care.
  • Ability to work independently and as part of a team.
